Youth Intensive Outpatient Program

  • 95% of clients avoided out-of-home placement and remained safely with their families

Health Homes Program

98%prevention rateOut of 568 clients in our Health Homes Program for youth with multiple chronic conditions or serious emotional disturbance (SED), 562 were able to remain with their families and avoid out-of-home placement.
